• The ADS Spring Horse Driving Trial (HDT) entry form pages include- announcer, catering, both ADS & CSHP disclaimer forms and fees. 

 REMEMBER THESE ADDITIONAL FORMS: 

• ADS Pony measurement card with pictures.  
• Flu shot receipt from vet.  Flu/Rhino per ADS regulations 
• Out of state equines require Negative Coggins and health certificates.

• Send above forms to our Show Secretary Listed below.  

•  Order Golf Cart reservations & Portable Stall reservations as early as possible. (there are limited amounts)  Contact Deb Packard  directly via txt/email.   
• Send Golf Cart form & separate check to Deb Packard.

• Driver  may compete in two divisions if schedule permits. Equines may compete  only once. 
• Second go in a single division is HC. 
• Grooms/navigators,  carriages may be shared only if schedule permits. (ADS article 917)  Please request on your event entry form.
• Results listed on ADS and Carriage Driving Today websites.
•  ADS golf cart rules apply-see below.
